Feedback and Solutions for Module Dead Lamp Issues

On March 11, 2026, we received 10 pieces of P4 outdoor modules [320*320(mm)] returned by the customer from the project site. The customer reported an issue of dead lamps on the modules, requiring our company to conduct root cause analysis and maintenance on the defective products as well as submit an 8D analysis report.

1. Case Overview

An order of 320×320mm outdoor P4 modules was delivered to South Korea on Nov 12, 2025. In March 2026, the customer returned 10 defective modules due to dead lamp failure. A cross-functional team consisting of QA, Sales, Production and After-sales was set up for root cause analysis via 8D report.

2. Defect Description

Visual inspection: Corner collision damage found on multiple modules.
Function test: Two types of dead lamp issues: single dead lamps around mounting holes and clustered dead lamps at module edges.

3. Root Cause Analysis

Single dead lamps near mounting holes: Improper packing operation caused external force damage to surrounding beads.
Edge clustered dead lamps: Rough on-site cabinet installation led to edge bumping and lamp damage.
All failures resulted from improper manual operation and external physical impact.

4. Corrective Actions

Temporary Action

Customer counts defective goods and sends them back to factory for repair; no relevant stock available.

Permanent Improvement

Marketing team produces standard operation videos for T-type maintenance wrench usage and module packing. Operators must align tools vertically with lock holes; tilted insertion and forced operation are forbidden to prevent lamp pin damage.

5. Effect Verification

A follow-up team with QA, production, sales and after-sales staff tracks the running performance of repaired modules at customer’s site and verifies long-term implementation of new operation standards.

6. Experience Summary

Besides raw material defects, improper packing and field installation causing physical bumping are major causes of outdoor module dead lamps. Standardized operation documents and regular training shall be implemented to prevent recurrence.
